This edition includes 214 tables and more than 900 references to publications or presentations made in 2010 and 2011, reflecting the authors' efforts to keep abreast of new developments by attendance at major scientific conferences and by systematic review of 42 relevant journals. Recommendations presented here are based largely on US federal guidelines for antiretroviral therapy for adults, pregnancy management, prophylaxis and treatment of opportunistic infections, management of occupational exposure, management of co-infection with tuberculosis, hepatitis B, and hepatitis C, prevention of transmission, and management of sexually transmitted infections. Based on recent advances there has been substantial expansion of and change in sections dealing with adverse reactions and treatment options following virologic failure. The authors have provided recommendations based on available data and personal experiences and opinions, as well as guidelines from authoritative sources. This is the 16th edition and was completed in October, 2011. Developments in treatment guidelines occurring after that time do not appear in this edition.
